CIBC Centre for Patients & Families
The vision of the CIBC Centre for Patients & Families is that patients and families move seamlessly from home to Vancouver Acute* (VA) facilities and back to home through effective navigation of the health care system.
To accomplish this we support patients and families to take an ownership role in their own health care so they can make timely, informed decisions.
Our Centre recognises that patients and families come from diverse communities and cultures**, and endeavours to provide information and assistance in an accessible and timely manner, adapting to the needs of various communities, where possible.
The CIBC Centre for Patients and Families is a centre of connectivity that has a dedicated focus on populations who face barriers when accessing health care and will assist all patients and families within VA to:
Access appropriate & reliable web-based consumer health information & resources
Access information abut Vancouver Acute* programs, services and clinics
Access information about health and other programs & services within their home community
Stay connected with family/friends while in the hospital
The CIBC Centre for Patients and Families also provides library services to VA employees and print health education materials (PHEM) support & development for Vancouver Coastal Health (VCH) employees.
* Vancouver Acute (VA) health service delivery area (HSDA) includes VGH, UBC Hospital, and G.F. Strong; it is one of the HSDA's within VCH
** Diversity includes the different gender, age, ethnicity, sexual orientation and gender identity, disability, socio-economic and geography mix that utilises VCH services.
